Upcoding Detection
Catches when a hospital bills a higher-complexity E/M code than the visit documentation supports. Example: billing a Level 5 ER visit (99285) for a 15-minute visit that warrants Level 3 (99283).
Duplicate Billing
Identifies when the same procedure, supply, or medication is charged more than once on the same date of service. A surprisingly common “mistake” in hospital billing systems.
Unbundling Violations
Detects when hospitals split bundled procedures into separate charges in violation of NCCI (National Correct Coding Initiative) edits. This inflates the bill by billing components individually instead of as a package.
Phantom Charges
Flags line items that have no supporting clinical documentation — charges for services, supplies, or procedures that appear inconsistent with the visit type and diagnosis.
Chargemaster Price Inflation
The compliance lever. Since January 2021, hospitals are federally required to publish their negotiated rates for every service (45 CFR Part 180). MedGuard downloads and parses these Machine-Readable Files (MRFs) to compare what you were billed against what the hospital itself filed with CMS for your specific payer and plan. When a hospital charges you $412 for a service they filed at $187, we catch it — and cite their own data in the dispute.
